**DOT Urine Drug Testing — $75** TruVision Verification Services (TVVS) provides professional DOT urine specimen collection services for employers and individuals participating in federally regulated workplace drug testing programs. Our DOT-Qualified Urine Specimen Collectors perform collections in accordance with applicable 49 CFR Part 40 requirements, including donor identification, specimen handling, Federal Custody and Control Form (CCF) completion, specimen sealing, and preparation for shipment to the designated laboratory. **COLLECTION SERVICES INCLUDE:** - DOT urine specimen collection - Donor identification and collection documentation - Federal Custody and Control Form (CCF) processing - Specimen sealing and secure handling - Preparation for shipment to the designated laboratory - Collection support for pre-employment, random, post-accident, reasonable suspicion, return-to-duty, and follow-up testing **SERVICE FEE — $75 PER COLLECTION** The $75 appointment fee covers one standard DOT urine specimen collection and associated collection documentation and specimen handling. Laboratory analysis, Medical Review Officer (MRO) review, and employer program administration are **not included** unless separately arranged and confirmed by TVVS. **APPOINTMENT REQUIREMENTS** Employers or authorized testing administrators must provide the required testing authorization, laboratory information, and applicable CCF instructions before the appointment. Donors must present acceptable identification and follow all required DOT collection procedures. **MOBILE & AFTER-HOURS SERVICES** Mobile and employer on-site collections may be arranged separately. Additional dispatch, travel, or after-hours fees may apply and will be disclosed before service.
